HEM Score = Current avg + ½ × weighted trend adjustment
(P + F + C + P + Q) ÷ 5 → most-recent month avg, adjusted by weighted trend

Rate each measure (1–5) for every month recorded. Each measure captures a distinct dimension of engagement.

Monthly Average: For each month, add all 5 measure scores and divide by 5 to get that month's engagement average.

HEM Score: Most-recent month's average, adjusted by half the exponentially weighted slope (recent months carry more weight). Clamped 1–5.

Score bands: 1.0–2.4 = Needs Attention | 2.5–3.4 = Moderate | 3.5–4.2 = Strong | 4.3–5.0 = Excellent
